Get referred
To qualify for this service, a young person needs to be aged 13-19 and using drugs.

This form provides the T2 youth worker with the information they need to help a young person at risk of developing problems with substance misuse.
Young people are usually referred with the help of people who work with young people, so, if you can, get a worker to help you fill out the form. People who might help:
- your local youth worker (find your local youth centre)
- a connexions adviser (find your local connexions centre)
- a specialist worker, e.g. at a homelessness centre, parents group, etc.
If you're having problems finding someone to help you fill out the form, or have any questions about it, please contact your local T2 worker.
